Intensive Case Management
If you have a mental health and/or addiction concern that is seriously affecting your life, this support helps you work with a case manager to find the right services in the community to aid your recovery, achieve your goals, and gain more independence.
Assertive Community Treatment Teams (ACTT) and Psychogeriatric Community Treatment Teams (PACTT)
If you have a serious mental illness, are experiencing the effects of psychosis, and have a history of hospitalizations, ACTT (for people 16 years and older) or PACTT (for people 55 years and older) can help you with your recovery by providing you with psychiatric treatment, rehabilitation, and support. The ACTT and PACTT are made up of health care professionals like case managers, nurses, employment specialists, social workers, peer support workers, addiction specialists and psychiatrists. To apply for either of these programs, the person referring you to this program must provide a detailed explanation in the “Reason for Referral” section of the online application to explain why you need this service.
Supportive Housing for People with Mental Health Needs
If you’re homeless or at risk of becoming homeless, have a complex mental health and/or addictions need, and/or dual diagnosis, or have a connection to the criminal justice system, there are several supportive housing options available:
Mental Health Supportive Housing
If you have a mental health concern and/or addiction, this program will link you to various levels of mental health and/or addictions support and housing options.
Addiction Supportive Housing
If you have severe substance use, are homeless or unstable in housing, and frequently use emergency services or interact with the justice system, this program will connect you to supportive housing.
Mental Health and Justice Supportive Housing
If you have a mental health concern and/or addiction, are homeless or at risk of being homeless, and are currently involved with the criminal justice system, this program will link you to supportive housing.
We are unable to provide exact wait times as they are based on programs and their availability. Once you are confirmed eligible you will be placed on a wait list for the services you have applied for.
Yes. Your eligibility for services will be decided after you apply, and your needs will be reviewed when you're referred to other programs.
Yes. Individuals must agree to the service.
Yes. If you live outside of the service Regions, you may apply for services if you are planning on moving to the area within the next 3 months. However, you will not receive service until you are living within York Region or South Simcoe.
Housing is for individuals with a mental illness and/or addictions who need support to live independently. Units are shared accommodation, are found throughout York Region, and are staff supported. The intensity of support may vary depending on your needs.

If you are already receiving services through Streamlined Access, you may be eligible for short-term support (30-90 days) depending on your circumstances. Eligibility is determined when your situation is assessed or when you are referred to this service.
