Case Management

YSSN’s caring and collaborative team will work with you to identify your goals, create a plan for the services you or your family need, apply for available services, and connect you to the right community resources and supports. Our goal is to help you achieve yours!

YSSN’s case managers assist children and youth with developmental delays, as well as adults with developmental disabilities, and their families in York Region. They help identify the services needed and connect them to community resources.

We can help connect you to:

• respite planning and camps
• special education services
• government funding resources
• health services
• children, youth and adult services

YSSN offers Resource Days to help families get answers, find information, and complete forms or applications. We can help you find services and supports, and get started on your goals.

Case management is also available if you need extra help with more complex problems or getting services.

For many families, Resource Days are enough to meet their needs.

After you have been referred to either Developmental Services for Children or for Adults, you may connect with Developmental Services Access any time for information and resources.
