Developmental Services Quality Assurance Case Manager 

One Year Full-Time Contract Position (with the possibility of extension or becoming permanent)

Primary functions include provision of assistance and support for adults and children who have a developmental disability and their families. Assist to navigate the service system and explore all community service options available to them. Tasks will include assessment of needs, development and coordination of service plans, referral support and skill development.

Position Requirements

  • Bachelors degree in Social Work or a related discipline (i.e. Psychology etc.)
  • Three years directly related front line experience
  • Experience working with individuals who have developmental disabilities is essential
  • Language skills and an understanding of cultural diversity are considered assets
  • Second language is considered an asset
  • Familiarity with the local service system and its providers is required
  • The successful applicant will be required to provide a police clearance that meets agency standards
  • A valid driver’s license, adequate insurance coverage for business purposes and regular use of a reliable vehicle.

Developmental Services Case Manager – Intake

One full-time permanent position

The Developmental Services Case Manager – Intake is responsible for the provision of case management and advocacy services to children and adults who have a developmental disability or dual diagnosis (developmental disability and mental health). The main functions include needs assessments using specific tools, eligibility screening for children’s referrals, warm transfers/referral support, and short term case management. Tasks will include assessment of needs, development of respite plans, telephone support, information gathering, wait list management and short term case management support.

Position Requirements

  • Bachelor’s degree in Social Work or related discipline (i.e. Health Sciences, etc.)
  • Three years directly related experience
  • Experience working with individuals who have developmental disabilities is essential
  • Language skills and an understanding of cultural diversity are considered assets
  • Familiarity with the local service system and its providers is required
  • A second language is considered asset
  • The successful applicant will be required to provide a police clearance that meets agency standards
  • A valid license, appropriate business use insurance and regular use of a reliable vehicle
